Summary

Provides safe and effective drug therapy and clinical patient care services to ensure optimal outcomes under the direct supervision of the respective preceptor. Ensures implementation of the pharmaceutical care plan by assuring the accurate and timely distribution of medications to patients. Serves as a clinical resource for other pharmacy personnel, medical and nursing staff, as well as to patients.

Essential Responsibilities and Duties

1. Reviews, evaluates, andprocesses medication orders for demographic data (e.g., gender, age, andweight), appropriateness of therapy, dose, frequency, route, dosage form,interactions, allergies, related laboratory values, and possible druginteractions.
2. Provides direct patient careby actively participating in rounds, reviewing patient charts, and developingand implementing patient - specific pharmacotherapy plan to ensure optimaloutcomes.
3. Ensures implementation oftherapeutic regimens and monitoring plans (care plans) by taking appropriatefollow-up actions.
4. Documents interventions andrecommendations/clarifications in the electronic Health Information System asapplicable.
5. Detects, analyses, and reportsadverse drug reactions.
6. Designs execute or participatein a research project that may include data collection, statical analysis, andwrite research papers.
7.  Participates/assists inconducting performance/quality improvement projects.
8. Actively participating inconducting medication use evaluation (MUE)
9. Provides information andeducation resource for patients, students, colleagues, medical andnursing staff, and other Healthcare professionals.
10. Participates in self andothers' education, training and development if assigned
11. Follows all Hospital’s relatedPolicies and Procedures.

Education

Pharm.D./Bachelor’sDegree in Pharmacy is required
