Summary
Contributes to the management, facilitation, and coordination of all aspects of patient care (i.e. history taking, clinical assessment, diagnosis, treatment and evaluation of care) with direct communication and collaboration with supervising physicians and surgeons.
Essential Responsibilities and Duties
Assesses, diagnoses, plans, implements, and evaluatestreatment/interventions and care.
Prescribes and reviews medication for therapeutic effectiveness,appropriate to patient needs and in accordance with evidence-based practice andinternational standards and protocols, and within scope of practice.
Identifies complications, confers with physicians for intervention andcommunicates patient status with physician/surgeon, and other involvedhealthcare professionals.
Partners with patients and families in order to support compliance withand adherence to prescribed treatments, and, provides information and advice onprescribed or over-the-counter medication on medication regimens, side-effectsand interactions.
Monitors surgical and medical complications, co-morbidities, infections,medication side effects, long term complication (i.e. recurrent disease).
Recognizes health problems and intervenes appropriately to assist thepatient in complex, urgent or emergency situations, including initiation ofeffective emergency care.
Reviews patient progress and adjusts plan of care as indicated.
Participates in department peer review and morbidity and mortalityreview.
Collaborates with nursing staff and interdisciplinary teams to establishcare plans, provides comprehensive documentation of progress and education,promotes prevention of long-term complications and improves the quality of life.
Coordinates discharge planning, demonstrates ability to completedischarge teaching and documentation.
Participates and contributes to the research activities includinginvolvement in data collection, dissemination and reporting.
Participates in the professional development and training of NursePractitioners.
Follows and monitors highest standards of professional and ethical codesof conduct.
Education
Master’s Degree in Advanced Nursing Practice / Nurse Practitioner is required.
Experience Required
Four (4) years of related experience is required.
